- How do I book an appointment?
- You may book your own appointment by calling 905-525-9140 x23575
- Is a referral required?
- Physician appointments require a referral. Please have your referring doctor fax a requisition to 905-526-7397. You will be contacted directly with an appointment date and time.
- A referral is not required for physiotherapy, massage therapy, osteopathy, dietitian or pedorthist appointments.
- Check with your insurance provider to see if a physician’s referral is required to process a claim for reimbursement.
- McMaster students do not require a referral to see a physician
- Are appointments covered under OHIP?
- Sport medicine physician and orthopaedic surgeon appointments are covered under OHIP
- Musculoskeletal radiology appointments are not covered under OHIP
- Physiotherapy, massage therapy, osteopathy, dietitian and pedorthic appointments are not covered under OHIP
- You may have coverage for all or part of these fees under one of the following plans: Student health insurance, Extended health insurance, Employee health insurance, Sport association insurance
- What should I wear to an appointment?
- For leg injuries, please wear shorts
- For shoulder and arm injuries, please wear a t-shirt or sleeveless shirt
- For back injuries, please wear movement accommodating clothing
- What do I need to bring to an appointment?
- For physician appointments, please bring a valid OHIP card
- Please arrive 15 minutes early to your first appointment to complete the required forms
- Do I need a prescription for a brace?
- Although a prescription is not needed to purchase a brace, your insurance provider may require one to process a claim for reimbursement.
- Is there HST on braces?
- Braces are tax exempt with a copy of a physician’s referral.
- Is parking available?
- Yes. Rebate vouchers are provided for paid services.
